Hana Yoshikawa  brings compassion and patience to the exam room and fully embraces One Medical Group's patient-centered approach to medicine. She emphasizes preventive health and holistic care and is especially interested in treating orthopedic injuries, infectious diseases and diabetes. After completing her undergraduate degree at UCLA and earning her master's from George Washington University, Hana gained experience as a physician assistant in primary care and emergency medicine in New York. What are your keys to a healthy lifestyle?

Being well is a lifetime commitment, so it’s important to  find ways to stay healthy that are not only feasible but also enjoyable. For most of us, stress is part of our everyday lives.  Nutrition, exercise, friends and family, and making the time to enjoy life are all key to warding off stress and to being well. The body is an amazing instrument – take good care of it and use it to its fullest potential!

What made you want to be a Physician Assistant (PA)?

Like many Physician Assistants, I started out as a pre-med, not knowing anything about the PA profession.  During that time I was also volunteering and working in different health settings, and met some amazing PA’s.  I was really impressed at their approachability, knowledge and skills.  The more I learned about the profession, the more I felt that it was the right choice for me.  I enjoy the flexibility that the career has given me to choose different specialties and to work closely with physicians and other health professionals.  I believe that practicing medicine for any clinician in this day and age is a collaborative process. The PA profession in particular nurtures the instinct to ask questions and pursue knowledge with a team approach.

 What do you feel you have the most knowledge about and what do you deal with the most?

Having worked in an emergency department in New York City for several years, I have seen many acute orthopedic injuries, and enjoy treating them. I also spent six years working in a community health setting working in geriatrics and in HIV medicine, which was a very rewarding challenge.  Seeing what can happen to the body with impaired immunity was key for me to understand illness and disease.  Working with that population, I learned a lot about infectious diseases and chronic illnesses.  Most importantly I learned about resiliency and how an individual’s commitment to being well can truly affect outcomes.  I also have a Masters degree in Public Health, which has broadened my understanding of preventive healthcare. 

What is your approach to overall healthcare?

I enjoy getting to know to know my patients and strive to understand how they perceive their own health and wellness.  From there I try to work with patients one on one to help them  life to  it’s fullest potential.  And while I am always there for my patients should illness arise, I do believe that an ounce prevention is worth a pound of cure!

1. What got you interested in medicine?

My dad is a physician. From early on, I had a fascination with his office, the potions he’d give people, the way in which his understanding of an unseen process could influence someone’s life.  I grew up in Holland, where when asked what you’d like to be when you grow up, you’d answer “I want to work for Doctors Without Borders” as often as you’d say “I want to be a firefighter.”  I think it was in my blood to want to help people!

2. How do you like working at One Medical compared to your past endeavors?

 

One Medical Group is completely dedicated to optimizing the patient’s experience, much more so than any practice I’ve worked in or observed.  I love the feeling of partnership with my patients, of providing guidance and a forum for discussion, without being dictatorial or paternalistic.  That sort of relationship was at the basis for the company’s formation and OMG has managed to really make it work.

3. I see you were the director of Health care for the Homeless, how do you feel about the homeless situation in SF?

Because I worked closely with homeless patients, I think it’s easier for me to see the person rather than the homelessness in someone living on the streets.  One key to reducing homelessness is to provide people with accessible mental health treatment and medical safety nets when the illness gets the better of the person. California is in a fiscal crisis, and when money dries up, in healthcare it dries up fastest for mental illness treatment. 

4. What are your keys to a healthy lifestyle?

Most importantly, food is your medicine.  Nearly every major illness that affects our society is modifiable or preventable through attention to nutrition.  Emotional stress management is also key to physical health; many illnesses arise due to inflammation in the body and external stress creates internal inflammation. The healthiest patients I have are those who pay attention to what’s in their stomachs and in their hearts.

5. Going to med school at The Ohio State University, you must be proud that your Buckeyes are number one in the nation right now in college football. Do you feel this is going to be their year to win the national championship?

Every year is the Buckeyes’ year! Go Bucks!

Steven Winiarski, DO, a graduate from the esteemed Chicago School of Osteopathic Medicine, is an experienced, board-certified Family Physician with additional expertise in osteopathic medicine. Why did you decide to enter the family medicine field?

I was exposed to the medical field at a very young age.  My mother was a pathologist and I used to accompany her to her lab and play with the myriad test tubes and paraffin wax used to store samples. I always had a passion for science. I entered college with the intent of pursuing a medical career However, after volunteering in hospitals I doubted whether the medical field would fulfill me.  I diverted from my original plan and enrolled into architectural school to follow in the steps of my grandfather.  After some time studying shapes, forms, themes, and spaces, I heard the world of medicine call me back.  

You do deal with a lot of sports medicine.  What is your sports history?

I grew up playing typical sports, including basketball, baseball, and soccer. In high school, I practiced martial arts for years and even went to

Japan

to compete. Currently I participate in road races and triathlons which gives me special insight into problems encountered by endurance athletes.

How often do you perform osteopathic manipulative treatments? What conditions are commonly treated with osteopathic manipulative treatments?

In my practice, manipulative treatments account for approximately twenty-five percent of routine treatments seen on a typical day. It could be higher but I make a concerted effort to treat a variety of conditions such as diabetes, hypertension, and skin diseases. 

Osteopathic manipulative treatments cover a wide range of illnesses and injuries.  My focus is identifying and treating mechanical issues that predispose a patient to injury.

What activities do you enjoy when you are not working?

            As mentioned above, I participate in triathlons and have taken up trail running since my move to the bay area.  I have never lived in a place more inviting to the trail runner!

What do you believe is the key to maintaining health?

In order to maintain health one must make health a priority.  It cannot be an afterthought.  Diet, exercise, and proper sleep form the foundation of good health.

What made you want to be a Physician Assistant?

Since childhood, my nurturing personality and interest in healthcare have guided me in the direction of medicine.  My mother, who is a pediatric nurse practitioner, also strongly influenced my decision.  I went through undergraduate in pre-med studies, and began looking into other options within the medical profession, which might allow me to have more of a flexible lifestyle.   Physician assistants have the ability to complete their education earlier, spend time with their families, yet, still enjoy the gratification that comes with helping and healing people.   After spending an entire summer shadowing PAs of myriad specialties, I realized that PAs have a surprising amount of autonomy, and therefore, being a physician assistant was the perfect career for me!

What is the difference between a PA and an MD?

Training for a PA parallels the medical education of MDs, except that it is a condensed version of the entire MD program, requiring an undergraduate and graduate degree.  A residency for sub-specialization is not required, which allows for more career flexibility.  PAs are able to have their independent patient relationships, prescribe all medications, and have more time for patient education.

What do feel you have the most knowledge about, what do you deal with the most?

Allergies and asthma, women’s health, and chronic disease management, such as hypertension, cholesterol, and diabetes, are a few of my interests.  I have asthma and allergies myself, so I can empathize with my patients on those issues. 

What are your keys to health, and what do you live by?

Take time to enjoy life.  Find activities you enjoy and make them a regular part of your daily schedule. 

Take an hour a day for yourself.

Find exercise that doesn’t  feel  like drudge work.  Dance!  Box! Do anything!  Just sweat!  

Remember…your mind and body always  work together.  One cannot function to it’s fullest without the other.  When you eat better, you feel better.  Be sure to get enough sleep!  Getting 8 hours of sleep per night is the best way to keep your mind and body healthy.

           What is the Biopsychosocial approach you employ in care of your patients?

 

    Most people only go to the doctor when their lives are affected by illness. I try to help the patients get back into their lives, what an illness may have taken away. I find out what a person likes or wants to do, and then I see how their current health status will either interfere or support them achieving their goals.  This involves looking at the illness itself and how it affects one’s life, and then trying to modify the disease and one’s life to create a balance that is tolerable and acceptable.

    This starts with education of the patient.  I enjoy educating the patient whether it is the first bit of education they receive, or expanding the knowledge of someone that has done extensive research. This starts with listening to my patients. As simple as that sounds, it is a common comment that I get from patients. “You are the first doctor that really listens to me” This is one of the things that is great about One Medical’s approach. The accessibility to and time with patients allows the doctor to listen, educate, and provide appropriate care during a visit and provide good follow up care when necessary. It’s very rewarding for the doctor and I think for the patients.

 

Your past experience was in a pain management clinic…what is your approach to health and treating patients?

 

Pain affects people’s lives in every single way. I utilize a multidisciplinary team approach in treating pain in order to improve function. My job is to be the centerpiece of a care team to help them decrease their pain and improve their function. This means that I assess the pain management needs of the patient and treat them utilizing  whatever means are appropriate. This may include surgery, medication, psychological care, chiropractic, acupuncture, etc

What is the trend you have seen in pain management.  Is, it leaning to new more drugs or away from that?

 

Pain management is now moving toward working on treating the brain. Neural plasticity work is the direction that healthcare/pain management is going. Neural plasticity is how the brain, central nervous system, and peripheral nervous system all interact to create certain pain states. Neural plasticity works on these pain states through movement, sound, visualization, smells, and more to create a new neural pathway to change how the brain interprets and deals with the pain.  Medications have a role to free the person to use their brain and body if the pain has overwhelmed their ability to do so.

 

 

What are your keys to a healthy life?

- Find and pursue what makes you happy.

- Be honest about what’s important to you.

- Take care of your body and mind so you can attain your goals.

- Minimize stress and maximize healthy habits.

Born in Pittsburgh, PA, Jennifer received her undergraduate degree in Journalism with honors from the University of North Carolina at Chapel Hill and received her MD at Georgetown University. Drawn to California, she completed her Internal Medicine training and an additional year of Hematology and Oncology at Stanford University. Jennifer believes that good health is best achieved when the patient and physician work together as a team. Through education and health information technology, she empowers her patients to take what they've learned and attain a healthy balance for life. She is board-certified in Internal Medicine and, in addition to providing comprehensive primary care, she offers patients with cancer an integrative approach.

 

Why did you decide to become a medical doctor?

When it comes down to it, ever since I was a young girl I really just wanted to help people and empower them. I was also able to be mentored by my neighbor who was an oncologist, which helped motivate me to become a doctor and advocate for cancer patients.

 

What is your approach to overall healthcare?

The patient and the doctor are partners in their overall health. I view myself as an advisor to the patient, and I devise practical treatment plans that they can follow and have them come back to update their progress.

 

How often should someone visit their primary care physician?

Well, there’s really no set frequency, but at minimum everyone should be getting annual physicals to check things like heart rate, blood pressure, address preventative care needs, and explore any abnormal things that arise.

 

How important is lifestyle decisions influence health?

Lifestyle is health. All the choices we make in our daily lives from what we eat to what activities we do, even how we manage our stress. It all shapes our lives. Anything I can do to help inform patients to make smarter lifestyle choices the better.

 

You have an integrative approach in treating cancer patients?

I did a year of fellowship in hematology and oncology at Stanford.  That fellowship offered me a window into the experience of a patient who finds out they have cancer. Now I feel   I am better equipped to be able to diagnose, treat, and most importantly coordinate the necessary open communication between  a cancer patients oncologist, hematologist, and me, the primary care physician. My patients can feel confident that all issues are being covered by the team.  When working with the cancer patients, I act as both their doctor and their advocate. 

 

 

What’s one of your biggest challenges in being a doctor?

There’s a fine line because you become attached to the patients as they go through the most traumatic thing they’ve ever gone through (cancer). You walk them through it, and sometimes they make it and sometimes they don’t.  It’s very difficult when they don’t pull through, but it’s through those experiences which have allowed me to be much more empathetic and understanding  than most doctors.

How do you incorporate your holistic training into your practice?

Well, my background is that  I’m Board Certified in Family practice and in Holistic medicine. Throughout my training, my goal has always been to figure out how to incorporate the innate healing of the body into my practice. The medical model doesn’t teach wellness, prevention, nutrition, etc. and they mainly focus on disease.   My holistic training helps me to teach my patients  the importance of a healthy mind and body as it relates to healing vs. simply treating the symptom with a drug.

What are your goals for healthcare?

I observe the big picture…using broader strokes to heal on a bigger level which includes exercise, nutrition, and life balance. I use functional medicine and try to get the body into balance. I look at the biochemical, biotransformation, detoxification, hormonal, immune, and inflammatory systems of the body, as well as try to understand the interconnectedness of symptoms...  For example, how does not getting enough  sleep affect the inflammatory reaction, natural cortisol production, and hormonal balance in the body.  I seek to treat the whole person…  and that’s when true healing happens.

Do you utilize any hands on techniques in your practice?

 I use various techniques and some are almost hands off healing such as healing touch, Reiki, energy work, etc. I mainly use it for things like stress reduction, anxiety, insomnia by using guided mediation and imagery. I use an individualized treatment program and clinical assessment for a person by figuring out what is going on with the person, and applying the appropriate healing modality specific for that person, instead of using a generalized approach.  I try not to do any test that won’t change my management of the patient and may be costly.

What would you consider to be the keys living a lifestyle of health and vitality?

1.       What you put in your body (nutrition)

2.       Exercise/movement

3.       Connection to loved ones (including yourself, friendships, family).

4.       Replenishing sleep

5.       Managing stress

What is your philosophy to living a healthy lifestyle?

 Life is all about balance and allowing yourself to enjoy being in balance, and allowing it to bring happiness and fulfillment into your life at every level.  Health is our natural state.
